I understand that while every precaution is taken to ensure the safety and well being of my dog(s), emergencies can occur.

In the event my dog becomes ill, injured or required immediate medical attention while in the the care of LEASH Loft & Spa, I authorize the team to see veterinary care on my behalf if I cannot be reached.

VETERINARY CARE

I authorize LEASH Loft & Spa to transport my dog to a veterinarian deemed appropriate, including the nearest available emergency veterinary clinic.

I understand that efforts will be made to contact me first using the information provided on file.

FINANCIAL RESPONSIBILITY

I agree to be fully responsible for any and all verterinary costs incurred as a result of emergency care, including examination, treatment, medication, and transportation.

I understand that payment may be required at the time of service and agree to reimburse LEASH Loft & Spa for any expenses paid on my behalf.

ACKNOWLEDGEMENT OF RISK

I understand that group environments and grooming services carry inherent risks, including minor injuries or illness.

I authorize LEASH Loft & Spa to act in the best interest of my dog in the event of an emergency.
